Gonioscopy and AS-OCT reveal structural changes in the anterior chamber angle post-GATT

Gonioscopy, combined with anterior segment optical coherence tomography (AS-OCT), is effective for assessing long-term structural changes in the anterior chamber angle following gonioscopy-assisted transluminal trabeculotomy (GATT), according to a study.

In the study, detailed gonioscopy and AS-OCT were used to examine the cleft and trabecular flap positions in 10 eyes at least 6 years post-GATT.

Results typically showed an open cleft in the anterior chamber angle with a visible trabecular flap. However, 3 different cleft appearances were observed: open, closed, and partially open. Over time, some trabecular flaps moved back toward the incision site, resulting in closed clefts on gonioscopy. AS-OCT confirmed that when the cleft was visible, it connected Schlemm’s canal to the anterior chamber. On average, the cleft remained open in 4.0 clock hours, mostly in the superior quadrants. Importantly, the degree of cleft openness did not affect the reduction in intraocular pressure.
